*Make up testing Friday, January 23rd at 6:30pm

Please wear full uniform and arrive 10 minutes before the testing time

I will hereby assume any and all losses, damages, and/or injuries I may sustain or injur, if any, while attending or participating. I hereby waive any and all claims against W Kim Tae Kwon Do and fellow participants of this promotional examination, individually, or jointly, for any losses and/or injuries. I also agree that any and all photographs taken by the officials of the promotional examination activities are the sole property of the promotional examination officials to be used as they wish.
